Library Services: Online Forms

Book or Media Purchase Request

The Library is very interested in receiving suggestions about books and feedback about our collection. If you would like to suggest a book or media for purchase use this form or contact Barbara Kobritz at ext. 4362.

Community Borrower Application

We welcome community residents to use our services. You can obtain a Community Borrower card by completing this online application and providing proof of identification to library staff. If you are completing it off-campus, we will contact you to verify your identification. Once approved, your new card will be mailed to you and you will be able to check out items. You can add money to your card for printing and photocopying in the Baker Commons or to use in the cafeteria or campus vending machines. Questions? Contact Lucy Yang, 607-844-8222, x4361.

Computer Classroom Reservation Request

The Baker Commons has a smart classroom with 24 student computers, which teachers can schedule for occasional sessions. Use this reservation form only to reserve a classroom. If you want a librarian or tutor to work with your class please go to Instructional Services for Faculty.
